Hamida Yaqoobi is a Nurse Practitioner FNP-C licensed in the state of New Jersey. She is a member of the American Association of Nurse Practitioners and has been practicing in healthcare for about 15 years. Yaqoobi graduated from Felician University and joined the National Registry of Certified Medical Examiners in 2020 (Medical Examiner# 1030935462). Her experience and education reflect a strong commitment to healthcare, providing her with the expertise to serve patients effectively.

Bring With You 
Exam

  • Valid Driver's License

  • Medical Records/Prescriptions/Medication Bottles

  • Clearance letter from Cardiologist if you've had a heart attack or heart bypass surgery. Also, bring documentation of a properly functioning pacemaker, if applicable.

  • Clearance letter from any surgeries within the last year.

  • Copies of previous blood pressure readings or statements from your Primary Care Doctor, especially if you have issues with blood pressure (e.g., white coat syndrome).

  • CPAP Report (at least 3 months of data) if you're using a CPAP machine.

  • Clearance letter to drive if you take a scheduled narcotic or amphetamine for any reason (e.g., ADHD, anxiety, depression, weight loss, etc.).

  • Clearance letter from pulmonologist if you have asthma and need approval to participate in sports or camp.

  • This will help ensure your physical exam goes smoothly!

What to Expect

DOT Exam: A comprehensive exam including vital signs and a non-drug urine test will be required for clearance.​

Sport and Camp Physical: A comprehensive exam with vital signs to ensure you are cleared to safely participate in sports or activities.

​

School Physical: A comprehensive exam with vital signs. Be sure to bring all immunization records if required.​

​

Pre-Employment Physical: A comprehensive exam with vital signs and completion of necessary forms.

  • Important note: We do not administer vaccinations or Mantoux testing.

Services are provided for individuals aged five years and older. Please arrive 15 minutes prior to your scheduled appointment and bring any required documentation for clearance. Please note that we do not conduct blood work, TB testing, or administration of any vaccinations.
